Company Overview

Shriners Hospitals for Children in Portland is a 40-bed pediatric orthopedic hospital that provides comprehensive care in a family-centered environment. Located on the campus of Oregon Health and Sciences University and overlooking downtown Portland and the Willamette River, the Hospital maintains a strong link with various teaching facilities and also conducts clinical and basic research in several areas. All services provided at the hospital are done so at no charge to patients and their families.

BENEFITS OVERVIEW

Insurance Eligibility
Employees:

• Full-time salaried or full-time hourly employees – scheduled to work at least 72 hours per pay period or
• Part-time benefit-eligible salaried or part-time benefit-eligible hourly employees – scheduled to work at least 40 or mores per pay period (but less than 72 hours per pay period)
Dependents: Eligible dependents include an employee’s legal spouse and unmarried dependent children until the age of 23.

MEDICAL INSURANCE: SHC-Portland offers the choice between two medical plans. Prescription is included under each medical plan
1. Blue Cross Blue Shield PPO Plan: This plan offers two levels of benefits. If you receive services from a provider who is in the PPO network, you receive the highest level of benefits. If you receive services from a provider who is not in the PPO network, you receive the lower level of benefits. In either case, you coordinate your own care. There is no need to select a Primary Care Physician (PCP). No referrals are needed for specialty care.

2. ODS Managed Care HMO: Members select a Primary Care Physician (PCP) from the ODS website. The PCP coordinates all healthcare needs including referrals, specialty care authorization, and hospitalization authorization. Each family member may choose a different PCP. All referrals to specialists need to go through your PCP.

DENTAL INSURANCE:
Aetna Dental Preferred Provider Organization (PPO) providers offer 100% coverage for preventative care, 80% for basic care, and 50% for major care.

VISION INSURANCE: SHC-Portland offers the choice between two vision plans.
1. VSP Access: Benefit is employer paid. As a VSP member, you will receive the 20% discount on your eye exam, frame, lens options and 15% discount on your contact lens exam. These discounts are only available from the VSP network doctor who provided your eye exam within the past 12 months.
2. VSP SUPPLEMENTAL INSURANCE: Exam and prescription lenses covered in full. Frame of your choice covered up to $120, plus 20% off any out-of-pocket costs. Contact lenses instead of glasses, your $120 allowance applies to the cost of your contacts and the contact lens exam (fitting and evaluation). You will get the best value from your VSP benefits when you visit a VSP network doctor.

LIFE AND ACCIDENTAL DEATH INSURANCE:
• Basic Life insurance: SHC provides basic life insurance coverage at no cost. Coverage is one time employee’s annual base pay, up to $750,000.
• Optional Life Insurance: You can elect optional life insurance coverage up to $150,000, up to $50,000 for spouse, and up to $20,000 for children.
• Optional Accidental Death and Dismemberment (AD&D) insurance: You can elect optional AD&D up to $100,000.

DISABILITY COVERAGE:
• Short-Term Disability (STD): All benefit eligible employees who have completed six months of continuous benefit eligible employment are eligible for Short Term Disability Benefits up to a maximum of 60% of base pay to a maximum of $1,615 per week. All costs of the plan are paid for by SHC.
• Long-Term Disability (LTD): Long Term Disability benefits are provided to all benefit eligible employees who have exhausted six months of Short Term Disability benefits and remain disabled. Employees must have completed one year of continuous benefit eligible service and must have been actively working at the time of the onset of the disability. LTD benefits replace up to a maximum of 60% of your base pay to maximum of $7,000 per month while you are injured or ill.

FLEXIBLE SPENDING ACCOUNTS (FSA): Contributions to be deducted before taxes
• Healthcare Spending Account: Employees can contribute up to $5,000 per year to pay out of pocket health care expenses, such as co-pays and deductibles.
• Dependent Care Spending Account: Employees can contribute up to $5,000 per year to pay dependent care expenses.

RETIREMENT:
• 403(b) Retirement Savings: The 403(b) Savings Plan has been established to provide retirement income and help employees save regularly to meet their long range financial objectives. Employee contributions are pre-tax. All employees are eligible to participate regardless of their classification of employment. Employee Contributions can begin immediately upon hire. Employees have the choice of investment options and make final determination of which fund(s), in which they will invest their money. SHC will match contributions by 50% up to 6% of an employee’s annual pay, after one year of service.


• Retirement Plan
The employee must complete five years of service with at least 1000 hours in each calendar year to be vested in the retirement plan. SHC pays the full cost of this benefit. Vested employees age 55 or older with intentions of retiring must give Human Resources a minimum of 90 days notice of retirement.

• 457(b) Deferred Comp Plans: All highly compensated employees (over $200,000) and a select group of key executives are eligible to participate. Employees must be approved by Corporate Headquarters before contributions can begin. This plan allows eligible employee to defer compensation up to current IRS maximum in a chosen fund(s). Contributions are solely employee funds and not matched by employer.

AFLAC: SHC offers two voluntary products through Aflac

• Accident: 24 hour coverage; Helps with deductible in the Emergency Department or Doctor’s office; Accidental Death and Dismemberment Benefit; Hospital Confinement Benefit per Day; Physical Therapy and Follow-up Care; Surgical Benefit; Wellness Benefit; Ambulance and Transportation Benefit.

• Hospital Confinement: Policy covers accident, surgery and sickness (including pregnancy); Covered person is admitted for inpatient care or outpatient surgery; Daily Confinement; Outpatient Surgery benefit; Rehab Unit Benefit; Invasive Diagnostic Exam; Surgical Benefit Inpatient/ Outpatient; Waiver of Premium.

TIME OFF:
• Paid Time Off (PTO): A newly hired employee will be employed with the accrual rate of 200 hours per year. After five (5) years of service, employees will automatically move up to the 240 hours per year accrual rate and after ten (10) years of service, employees will automatically move up to the 280 hours per year accrual rate.
• Extended Illness Bank (EIB): All full-time employees will accrue EIB hours at the rate of forty-eight (48) hours per year. EIB hours will accrue on a pro-rated basis for part-time employees that are normally scheduled to work twenty (20) or more hours per week. EIB hours are accumulated from the first day of employment, but may not be utilized during the initial three (3) months of employment.

SERVICE AWARDS CELEBRATION PROGRAM: SHC employees will be recognized for their fifth year of service and for every five years thereafter.

EMPLOYEE ASSISTANCE PROGRAM (EAP): SHC Employee Assistance Program offers this program to all employees and their families at no cost. The provider is Magellan Health who provides problem identification, counseling, and referral services. Counselor are experienced in providing support, understanding and guidance for a broad range of needs, including: family/relationship/parenting issues; alcohol and drug dependencies; emotional and stress problems; adjusting to change; child and elder care; grief; self-improvement; conflicts at work or home; pre and postnatal concerns; work/life balance; and so much more.

EDUCATIONAL OPPORTUNITIES:
• Tuition Reimbursement: Tuition Reimbursement is available to all benefit eligible employees who have completed at least 6 months of continuous service. Reimbursement is 80% of tuition and books
• Continuing Education: In addition to the tuition reimbursement program, SHC encourages continuing education for its employees in an effort to improve their knowledge and skills by providing time and financial resources.

BEREAVEMENT LEAVE: All benefit eligible employees may be permitted up to 24 hours benefit pay pro-rated by your assigned FTE, for the death of an immediate family member. An immediate family member is defined as spouse, child, parent, sibling (brother/sister), grandparent, grandchild, corresponding in-law (grandparent, mother, father, sister, brother), and corresponding step relation (grandparent, mother, father, sister, brother.

JURY AND WITNESS DUTY: If you are a benefit eligible employee who is summoned to jury duty, SHC continues your salary during your active period of jury duty for up to a maximum of fifteen working days per calendar year.


CREDIT UNION MEMBERSHIP: Employees have access to credit union memberships with Unitus Community Credit Union.
